Respiratory dysfunctions in patients with Parkinson's disease (PD) are common and are associated with increased mortality and morbidity. Respiratory dysfunctions have been described in patients with PD since the first observations of the disease by James Parkinson in 1817. Respiratory dysfunctions are multifaceted; obstructive ventilatory dysfunction includes upper airway obstruction (UAO), restrictive ventilatory dysfunction, respiratory muscle weakness, abnormalities in respiratory rhythm genesis, kyphosis, aspiration pneumonia, and side effects of medication. Many of the respiratory symptoms occur in the early stages of the disease. Respiratory dysfunction may predate motor symptoms by several years. Patients with PD usually do not complain about respiratory symptoms due to their sedentary lifestyle. The mechanisms of respiratory dysfunction in PD are evolving. This review will discuss various respiratory abnormalities that are associated with PD.
